About The Position
Hamilton Health Center, a mission-driven Federally Qualified Health Center (FQHC), is seeking a Family Medicine Physician to provide high-quality, patient-centered care to communities in Millersburg and Newport, Pennsylvania. This is a full-spectrum outpatient primary care opportunity, serving patients across the lifespan—from pediatrics to geriatrics. Our model is rooted in whole-person care, with integrated behavioral health, social services, and care coordination support, allowing providers to focus on meaningful, outcomes-driven care. This role offers the opportunity to practice medicine with purpose—serving underserved populations while working within a collaborative, team-based environment.
Requirements
- MD or DO from an accredited institution
- Board Certified or Board Eligible in Family Medicine
- Active Pennsylvania medical license (or ability to obtain)
- Strong communication and interpersonal skills
- Proficiency with EMR systems
Nice To Haves
- Experience in an FQHC or underserved population setting
- Knowledge of HRSA and Joint Commission standards
- Bilingual (Spanish, Nepali, French, or Arabic)
- Prior leadership or supervisory experience
Responsibilities
- Provide comprehensive primary care services, including preventive care, chronic disease management, and acute care
- Perform full physical examinations and annual health assessments
- Conduct basic gynecologic exams as part of routine care for adult female patients
- Diagnose and manage medical conditions, coordinating care as needed
- Maintain accurate, timely, and compliant documentation in the EMR system
- Collaborate with behavioral health, care management, and support staff to deliver integrated care
- Participate in quality improvement initiatives, peer review, and case management discussions
- Contribute to on-call coverage per provider schedule
- Attend staff meetings, trainings, and required in-service sessions
- Provide supervision to Nurse Practitioners and Physician Assistants, as applicable
- Ensure compliance with HIPAA, OSHA, and organizational standards
