About The Position

Hamilton Health Center, a mission-driven Federally Qualified Health Center (FQHC), is seeking a Family Medicine Physician to provide high-quality, patient-centered care to communities in Millersburg and Newport, Pennsylvania. This is a full-spectrum outpatient primary care opportunity, serving patients across the lifespan—from pediatrics to geriatrics. Our model is rooted in whole-person care, with integrated behavioral health, social services, and care coordination support, allowing providers to focus on meaningful, outcomes-driven care. This role offers the opportunity to practice medicine with purpose—serving underserved populations while working within a collaborative, team-based environment.

Requirements

  • MD or DO from an accredited institution
  • Board Certified or Board Eligible in Family Medicine
  • Active Pennsylvania medical license (or ability to obtain)
  • Strong communication and interpersonal skills
  • Proficiency with EMR systems

Nice To Haves

  • Experience in an FQHC or underserved population setting
  • Knowledge of HRSA and Joint Commission standards
  • Bilingual (Spanish, Nepali, French, or Arabic)
  • Prior leadership or supervisory experience

Responsibilities

  • Provide comprehensive primary care services, including preventive care, chronic disease management, and acute care
  • Perform full physical examinations and annual health assessments
  • Conduct basic gynecologic exams as part of routine care for adult female patients
  • Diagnose and manage medical conditions, coordinating care as needed
  • Maintain accurate, timely, and compliant documentation in the EMR system
  • Collaborate with behavioral health, care management, and support staff to deliver integrated care
  • Participate in quality improvement initiatives, peer review, and case management discussions
  • Contribute to on-call coverage per provider schedule
  • Attend staff meetings, trainings, and required in-service sessions
  • Provide supervision to Nurse Practitioners and Physician Assistants, as applicable
  • Ensure compliance with HIPAA, OSHA, and organizational standards
