Job Overview: The physician provides comprehensive acute, chronic, preventive, curative, and rehabilitative medical care across specialized areas, including family medicine, internal medicine, and geriatrics. With expertise in transitions of care, chronic disease management, advanced care planning, and palliative care, this role supports patients throughout the continuum of care.

Requirements

Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree.
Completed a Family Medicine/Internal Medicine residency at an accredited medical institution.
Active and Unrestricted California Medical License and Active DEA License.
Board certified or board eligible.

Responsibilities

Developing and implementing clinical protocols
Evaluating and managing both acute and chronic conditions
Coordinating referrals and hospital follow-ups
Supervising clinical support staff
Contributing to interdisciplinary care planning
Maintaining detailed, timely documentation in compliance with clinic, state, and federal standards
Active participation in quality assurance
Continued professional development
Participation in internal meetings
