About The Position

United Health Services (UHS) is seeking a board certified Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) for our UHS Upper Front Street Medical Center- Same Day Clinic Provider position. This is a busy, Family Medicine out-patient practice in a well-established office and we are looking for a Family Medicine provider to handle same day appointments and wellness visits as needed for infants to adult patients. This position is 100% outpatient. Office hours Monday- Friday 8:00 a.m. – 5:00 p.m. with no call. Primary Department, Division, or Unit: UHS Primary Care Upper Front Street Annual Compensation Range: $131,900 - $155,200

Requirements

• Master of Science in Nursing (MSN) or Physician Assistant degree
• Possession of an NPI (National Provider Identification) number, as evidenced by the receipt of a copy of the notification assigning the number
• Current NYS Registered Professional Nurse or Physician Assistant License
• Documentation of current CPR certification required within one (1) week of hire, with maintenance of current CPR certification during employment
• Certified Prescriptive Privilege in NYS

• Possession of Bureau of Narcotics and Dangerous Drugs (BNDD)Number
