About The Position
Family Medicine Nurse Practitioner or Physician Assistant Step into a collaborative, relationship-centered practice at our Albany Clinic, where you’ll join a supportive team of Physicians and Advanced Practice Providers committed to exceptional primary care. In this role, you will share and grow a patient panel with a physician partner and have the option to incorporate same-day care if interested. If you value teamwork, continuity, and building long-term patient relationships, this is a fantastic opportunity to make a meaningful impact. Schedule: 0.6–1.0 FTE, Monday–Friday, clinic hours approximately 8:00 a.m.–5:00 p.m. (flexible based on candidate interest)
Requirements
- Certified as a Nurse Practitioner
- Current Minnesota Physician Assistant Licensure
Nice To Haves
- Previous experience in Urgent Care
- Previous experience Family Medicine/Walk-In Clinic
Responsibilities
- Provide comprehensive preventive care for children and adults
- Support patients in managing chronic conditions
- Develop and coordinate treatment plans based on clinical assessments
- Review labs and determine the appropriate next steps in care
- Partner with patients to promote wellness, education, and long-term health
