About The Position

CentraCare is hiring a full-time Family Medicine Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our team at the Melrose clinic. As a provider developing your own patient panel, you will independently see patients of all ages for acute and chronic care needs. Our CentraCare – Melrose clinic providers diagnose and treat short-term illnesses; provide preventive care including routine checkups, health-risk assessments, immunizations and screening tests; offer personalized counseling on maintaining a healthy lifestyle; and manage and coordinate care for those with chronic illnesses. The Melrose clinic has access to lab and imaging services during regular clinic as well as extended hours.

Requirements

• Certified as a Nurse Practitioner or hold a current Minnesota Physician Assistant Licensure and be eligible for Minnesota licensure.
• Must have a current American Heart Association BLS Healthcare Provider card.
• Must have a Drug Enforcement Agency (DEA) registration number prior to receiving clinical privileges.
