Advance Community Health is seeking a compassionate and mission-driven Family Medicine Physician to join our growing team supporting our locations in Wake Forest and/or Louisburg. As a Federally Qualified Health Center (FQHC), we are dedicated to delivering high-quality, accessible care to a diverse and underserved patient population. Requirements

  • MD or DO from an accredited medical school.
  • Board Eligible or Board Certified in Family Medicine (Required).
  • Completed accredited residency training (Required).
  • Active (or eligible) North Carolina medical license.
  • Active DEA and NPI (Required).
  • Current BLS certification.

Nice To Haves

  • Experience with Epic Systems (preferred).
  • Bilingual (English/Spanish) strongly preferred.

Responsibilities

  • Provide comprehensive primary care services to patients of all ages.
  • Deliver preventive care, chronic disease management, and health maintenance.
  • Exercise independent clinical judgment in diagnosing and treating patients.
  • Order and interpret diagnostic tests, prescribe medications, and develop care plans.
  • Educate and counsel patients and families on health conditions and wellness.
  • Collaborate with a multidisciplinary care team to ensure coordinated patient care.
  • Refer patients to specialists and other healthcare services as appropriate.
  • Accurately document care in the electronic medical record (EMR).
  • Ensure appropriate coding for billing and compliance.
  • Supervise and support clinical staff as needed.
  • Uphold the highest standards of medical ethics and patient-centered care.
