About The Position

Family Development Coach provides case management and related workshops. The FDC is the primary contact for fathers participating in the program. The FDC will contact each assigned participant at least once every two weeks to assess progress to achieving goals for the Individual Service Plan (ISP). Connect participants to services that address immediate needs related to housing, substance abuse, physical and mental well-being, violent or risky behavior, and other areas that threaten to inhibit full participation and ability to achieve personal and program goals.

Requirements

  • Bachelor’s degree in social work or a related field (minimum)
  • Experience working with City systems such as public assistance, child support, child welfare, education, and housing.
  • Associate’s degree and completion of the Family Development Training (or other credentials identified by DYCD) and experience working with City systems such as public assistance, child support, child welfare, education, and housing.
  • Completion of the Family Development Training within one year of being hired (or other credentials identified by DYCD)
  • At least three (3) years successful experience in providing case management/counseling and working with City systems such as public assistance, child support, child welfare, education, and housing
  • Good verbal and written communication are essential
  • Computer proficient

Responsibilities

  • The Family Development Coach will complete intakes, assessments, individual service plan (ISP) and the basic summary. Priority will be given to the completion of the Pre-and-Post Parenting Skills Knowledge survey to ensure the program attains the contracted number of outcomes each year.
  • Utilize a case management model for participants in all tiers, which would include the following 4 components: (1) holistic assessment of the father’s strengths and needs and development of an Individual Service Plan (ISP) listing supportive services, benefits and resources needed; (2) implementation of the ISP (coordinating community resources and working with and advocating on behalf of the family to meet identified goals); (3) regular review of the ISP to assess whether goals have been met or need to be changed; and (4) follow-up to ensure that the father has received requested services.   • The Fatherhood Initiative will deliver eight required activities: 1. Orientation, 2. Parenting Skills Training, 3. Supervised Visitation Coaching, 4. Case Management, 5. Community Projects, 6. Support Groups, 7. Family Events, and 8. Workshops.
  • The Fatherhood Initiative will serve and customize services for fathers with varying levels of child visitation, coparent engagement, court and agency orders, skills, and needs. Fatherhood will offer four service tier options to participants. The Fatherhood Initiative will implement parenting skills training to participants with two different curricula: ProFatherhing15 and Supporting Father Involvement (SFI).
  • Guided by DYCD’s Case Management Toolkit, develop goal-oriented Individual Service Plan (ISP), and facilitate applications and referrals to various services and supports. Address the changing needs of fathers and adapt services accordingly.
  • Provide individual, family and group counseling, provide direct services/case planning to clients including mediation/conflict resolution training to fathers to facilitate effective co-parenting and visitation. When necessary, Case Managers will assist with supervised or monitored visitation opportunities for fathers.
  • Assess father’s immediate needs and current child support order and visitation/placement arrangements.
  • Maintain contact with program participants at least once every two weeks and follow-up contact for program alumni. (Active caseload of 20-24)
  • Build effective internal and external working relationships with key stakeholders.
  • Enter progress notes in DYCD data system of record.
