About The Position

The Family Connections Consultant at Easterseals Blake Foundation provides family-centered home visitation services aimed at improving the safety and well-being of at-risk families and their children. This role involves comprehensive assessments, crisis intervention, and the development of individualized family plans in collaboration with the Department of Child Safety (DCS) and other support systems. The consultant works to enhance family functioning, increase parenting competencies, and stabilize family units to prevent children's removal from their homes.

Requirements

• Bachelor's degree in social services or a related field.
• 1 year of work-related experience providing behavioral health assessments and interventions.
• Experience working with at-risk families and children.
• Valid, unrestricted Arizona driver's license and willingness to use personal vehicle for work.
• Current State of Arizona Fingerprint Clearance card.
• Current First Aid and CPR certifications.
• Bilingual in Spanish and English preferred.

Nice To Haves

• Experience in child and family counseling.
• Motivational interviewing skills.
• Strong communication and negotiation skills.

Responsibilities

• Respond to referrals from DCS within established timelines and provide immediate crisis stabilization and intervention as needed.
• Complete comprehensive, family-centered assessments using clinical observations and formal assessment tools.
• Document family strengths, resources, priorities, and concerns in reports.
• Conduct weekly visits and complete timely reports as per contractual requirements.
• Develop Family Service Plans with goals and strategies to meet identified outcomes for families and children.
• Work collaboratively with DCS workers and other support individuals to address family outcomes.
• Provide crisis intervention, individual, family, marital, and group coaching, and parenting education.
• Link families to community resources including behavioral health services, food banks, and other organizations.
