About The Position

The Family Case Planner will work in a child welfare prevention program that utilizes the Solution-Based Casework model to stabilize families with children under 18 at risk for placement outside their homes. Preventive services aim to stabilize and provide support so that children can remain in their homes.

Requirements

  • BA/BS/BSW required; master's degree preferred.
  • Excellent writing, communication and organizational skills.
  • Excellent interpersonal skills.
  • Flexibility and ability to work under pressure and meet deadlines.
  • Computer literate (CONNECTIONS and PROMIS proficiency a plus).
  • Two years of FTC or similar experience in child welfare is preferred
  • Exceptional mediation skills to effectively facilitate group conference situations.
  • Strong clinical skills and strength-based perspective.
  • Strong verbal and writing skills.
  • Excellent organizational skills
  • Ability to handle multiple tasks at one time.
  • Knowledge of core principles and standards for NYC Child Welfare system.
  • Ability to hold conferences in family’s homes as needed

Nice To Haves

  • Group skills training and experience a plus.
  • Bilingual (Spanish/English) a plus.
  • Computer literate (CONNECTIONS and PROMIS proficiency a plus).
  • Two years of FTC or similar experience in child welfare is preferred

Responsibilities

  • Direct Services are provided to clients via casework, individual, family, and group counseling, as indicated by individual case assessments.
  • Assist in Intake and Crisis Intervention
  • Conduct home visits and attend client-related meetings as needed.
  • Develop client assessments and service plans following the model outlined by Improved Outcomes for Children, Family Team Conference, and SBC.
  • Monitor and assist in service provision to all program clients as required by program mandates.
  • Maintain current case records and service plans (FASP) for families served utilizing Connections and PROMIS systems.
  • Participate in weekly supervision, case conferences and Family Team Conferences as directed by Supervisor and Program Director.
  • Provide parent education and other support services utilizing group format as determined by client need.
  • Participate in training appropriate to the development of enhanced Social Work skills as required by Children’s Services and HeartShare St Vincent’s Services.
  • Assist in day-to-day operations as required by the Program Director.
