About The Position

The Family Care Manager (Behavioral Health) provides care management to families caring for children, ensuring their physical, social, emotional, cognitive, and behavioral health needs are met. This role involves addressing challenges through individualized support, education, coaching, and resource referrals, empowering families to navigate community resources effectively.

Requirements

  • Bachelor's degree in Social Work, Social Science, Child and Family Sciences, Psychology, or a related field.
  • Valid driver's license with a driving history that meets Cherokee Nation underwriting requirements.
  • Knowledge of spreadsheet and word processing software.

  • Preference for speakers of Cherokee, Spanish, or American Sign Language.

Responsibilities

  • Provide health care services to individuals eligible for services according to the self-governance compact and funding agreement.
  • Advise individuals and families on issues including mental health, poverty, unemployment, and substance abuse.
  • Conduct individual and family interviews to assess situations and determine required services.
  • Perform approved screenings and assessments to identify client strengths and needs.
  • Arrange for support services such as child care and parenting classes.
  • Serve as a liaison between families and various service providers to facilitate support.
  • Develop and review service plans with families and conduct follow-ups on service quality.
  • Provide support and counseling to parents using an approved parenting curriculum.
  • Refer clients to community resources for various services and provide guidance on accessing them.
  • Maintain case history records and prepare reports in compliance with regulations.
  • Consult with parents and school personnel to address issues like truancy and misbehavior.
  • Conduct social research to stay informed about child wellness and community resources.
  • Facilitate processes with families to identify strengths and needs, providing emotional support and resource access.
  • Educate families on laws, policies, and procedures, empowering them to articulate their needs effectively.
  • Maintain local resource information for families and educate them on resource research.
  • Advocate for families to ensure proper service delivery and respectful treatment.
