About The Position

Join Southcoast Health, a not-for-profit, charitable health system with multiple hospitals, clinics, and facilities throughout Southeastern Massachusetts and Rhode Island. We are searching for a talented Family Advocate to be part of our New Beginnings Program, which began in 2016 and is grant and philanthropically funded, with operations supported through June 30, 2030. This position is based at the Southcoast Business Center in Fairhaven, MA, and follows a hybrid schedule. The Family Advocate will work collaboratively with a multidisciplinary care team to provide culturally appropriate, trauma-informed, person-centered care to pregnant and postpartum parents exposed to substances, including opioids, stimulants, and alcohol. The role involves ongoing care management for up to 3 years postpartum, focusing on health and wellness through education, advocacy, and resource linkage.

Requirements

  • Completion of 4 years of high school plus additional specialized courses or training is required.
  • A minimum of one year related work experience required.
  • Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
  • Solid knowledge of community resources and effective teaching/coaching skills.
  • Must possess and demonstrate highly effective interpersonal, customer service, and communication skills.
  • Must be fully vaccinated against seasonal Influenza and the COVID-19 virus or be exempt from the requirement.

Nice To Haves

  • Bilingual skills preferred.
  • Experience working with families and children and/or substance abuse treatment clients preferred.
  • Certificate in substance abuse counseling preferred.

Responsibilities

  • Provide culturally appropriate, trauma-informed, person-centered care to pregnant and postpartum parents.
  • Identify medical and social needs of clients and provide education, advocacy, and support.
  • Link clients to community resources to promote optimal outcomes for parent and baby.
  • Collaborate with a multidisciplinary care team to support programmatic grant funding.
  • Manage ongoing care for clients for up to 3 years postpartum.
