This dynamic, innovative, educational, and non‐invasive health care ministry is promoted through a collaboration between THR and area faith communities with an intentional focus on engaging, empowering and equipping congregations to support congregational faith and health ministries. The Faith Community Nursing program serves under and aligns with the larger Community Health Ministry (CHM) strategy of the Texas Health Community Engagement and Advocacy (CEA) Department. Roles are active with all members of CEA in creating North Texas faith group partnerships across the Texas Health 16 county service area, with special and intentional attention paid to Community Health Needs Assessment (CHNA) identified areas in order to address health equity needs. Operationalizes the Faith Community Nursing and Health Promotion Program Demonstrates an understanding of and practices within the ANA Scope & Standards of Faith Community Nursing Collaborates with Senior Director of Faith and Community Health and team to develop and implement an operational plan consistent with CHI Department objectives and the THR strategic plan ◦ Contributes to the program’s mission statement, program vision, goals, Standards of Practice, policies and procedures in collaboration with THR strategic plan and Community Health Needs Assessment (CHNA) findings Oversees FCN/HP program at multiple THR entities, collaborating with entity leadership and Pastoral Care manager to assess spiritual care needs of staff and develop education programs with an emphasis on the care of the spirit Oversees FCN liaison at each entity o Orientation of new liaisons o Involves entity liaison in building internal and external relationships and integrates the liaison’s input into the program o Acts as a resource in development of the liaison role and encourages liaison professional growth Actively participates in all FCN meetings, including department, entity, and corporate leadership levels Creates and sustains relationships with FCN/HP’s at local faith communities and faith-based outreach programs Creates and maintains the FCN/HP Letter of Agreement and Congregational Letter of Agreement Evaluates the FCN/HP program through collection of data and creation of annual faith community activity reports of faith community activity. Implements changes as appropriate: o Monitors monthly reports of faith community activity o Compile monthly and annual aggregate reports by entity o Utilize SharePoint/dashboard reporting tools o Collects and reports outcome data for FCN/HP activities Communicates appropriate information to program senior director via monthly reports, 1\:1 check-in, and monthly expense reports Identifies and deploys client educational program priorities specific to FCN/FCHP community programs/ministry Utilizing customer feedback from faith community nurses, faith community health promoters, faith leaders, CHNA, Texas Health customer feedback, congregational health interest surveys and/or community partners, and CHI initiatives to identify education needs and gaps Assess material and resource needs to meet identified educational opportunity o Identify existing resources from credible sources that can be utilized to meet educational needs. Examples include but are not limited to American Heart Association, American Diabetes Association, NAMI, CDC etc. o Adapt materials to meet literacy levels and cultural needs of the community being served o Develop and create educational resources that meet identified needs of the community when needed. Examples include – 1-page topical information; seminar content; in person presentation/classes Plan details for educational program implementation to include: o Training needs for educators (train the trainer) o Outcome measures o Tools to provide effective and efficient education that literacy and cultural needs Participates in curriculum development for faith community nurses, faith community health workers, faith community leaders, and community partners who work to improve the health and well-being of the community they serve Assesses and analyzes current trends in healthcare, anticipates changes in educational priorities, and communicates trends to FCN/HP community Participates in curriculum development for faith community nurses, faith community health workers, faith community leaders, and community partners who work to improve the health and well-being of the community they serve Assesses and analyzes current trends in healthcare, anticipates changes in educational priorities, and communicates trends to FCN/HP community Provides topic specific program instruction as faculty, supporting all FCN/HP courses including Foundations of Faith Community Nursing, and Fundamentals of Faith Community Health Promotion Writes curriculum to support identified continuing education needs that align with CHNA findings Develops and updates orientation curriculum and writes topic specific preventative health education programs Participates with the entire team in the development of curriculum, thereby, attaining valuable insight and assuring positive outcomes Support quality patient care outcomes Plan and manage assigned community clinic processes to improve performance. Facilitate delivery of quality community education curriculum Identify outcome measures for activities that demonstrate health outcomes (body, mind, and/or spirit). Ensure FCNs and FCHPs know how to measure and document to show program effectiveness Emphasize documentation expectations that meet credentialing standards/policy/guidelines/standards of nursing practice o Trains covenant FCNs and FCHPs in the use of provided electronic documentation Actively engages in quarterly data collection that demonstrates programs’ alignment with and progress toward CHNA-driven CHI health equity goals and objectives o Works with community FCNs and FCHPs to ensure reporting of activities and programs in the Henry Ford Documentation System o Completes monthly and annual reports of covenant congregation activities and patient interactions Demonstrate positive supportive role in change process Facilitate delivery of quality community education curriculum Emphasize documentation expectations that meet credentialing standards/policy/guidelines Demonstrate positive supportive role in change process Identifies opportunities for research o Supports research projects through development of research steps and project proposals o Assists with research activities and projects across the CHI department o Serves as subject matter expert as appropriate Serve as clinical expert/consultant across the Community Health Improvement Department Serve as subject matter experts Prioritize and Support the Community Health Needs Assessment (CHNA) processes Utilize CHNA findings to prioritize and inform faith community partners to drive deployment of programs and interventions that support needs identified by community members Utilize CHNA findings to determine program development and creation targeted to meet the needs of underserved communities Support program development through serving as subject matter expert (as appropriate) and creation of educational materials and curriculum development Actively participate in CHI Regional meetings to o Collaborate across CHI divisions to collectively address CHNA priorities o Support regional annual OKRs and contribute to achieving goals set by the regional team o Serve as connector to faith communities and faith-based organizations Support recruitment of community members/participants for programs across CHI Department through connections with faith communities and faith-based organizations Actively participate in CHI Department and divisional meetings Participate on committees across CE&A division and serve as subject matter experts as needed Support Program Growth and Development Actively seek relationships with faith-based organizations and congregations for the purpose of developing FCN Covenants and/or non-covenanted CHM partnerships Actively works with faith group leadership/council to gain the group’s ownership and committed support to health and health equity programs through FCN/HP Support Community Health Improvement Department initiatives that provide and expand growth in underserved communities with prioritization of identified high-needs zip codes Participates in identifying the volunteer FCN/HP for the community, when requested Develops and maintains relationships with affiliated faith communities and their leadership through appropriate communication channels Develops and maintains relationships with affiliated community outreach programs Provides mentorship to FCNs/FCHPs/Faith Leaders/Community Partners: o Implements and supports congregational Health Interest Survey in faith communities o Trains and supports documentation of faith and health ministry events through electronic documentation tool o Assists the FCN/HP with the development, implementation, and evaluation of the faith and health programs within the congregation, including site visits as needed o Plans and executes networking meetings and relevant continuing education of FCN/HP’s, coordinates meeting agendas and provides overall leadership and facilitation of meetings o Participates in congregational faith and health events, when requested Connects faith communities with community resources Promotes the Faith Community Health and Promotion Program internally and externally o Creates promotional articles, flyers, brochures to support THR’s Faith Community Nursing and Health Promotion program. o Creates public preventative education materials and clinical education materials for the promotion of FCN/HP practices. o Presents public education courses in collaboration with community affiliates. o Promotes THR FCN/HP program at an international, national and local level through publications, research, presentations and education. o Communicates outcomes and related stories to support internal and external communication i.e. CHI dashboards, community newsletters, entity reports Professional Qualities and Accountability Communication and Collaboration o Collaborates with relevant personnel in decision making processes; makes quality decisions and demonstrates willingness to act Provide Leadership and Oversight for Nursing Operations and Clinical Practice in programs through Faith Community Nursing and Health Promotion Staffing Community Clinics/Events Sustainability/Budget/Financial Responsibilities Identifies and evaluates program budgetary requirements and outcomes based upon congregational requests and identified needs Identify labor and non-labor resource needs during annual budget process Contributes to preparation of labor and non-labor operating and capital budgets for areas of responsibility Manage expenses within budget and provide justification for variances Maintain records of program financial expenses and report as needed to determine program needs and growth Report monthly data from program/community activities Maintains expert knowledge of the financial impact of the FCN program o Report monthly and annual cost avoidance and savings that result from care provided in the community o Grant Administration Participates in the acquisition of grants for the FCN/HP Program.
