The FACT-IL / MFT Support Nurse (LPN/RN) provides treatment, rehabilitation, clinical case management, care coordination, and support services to individuals served by the FACT-IL program. Additionally, the nurse completes nursing assessments within scope of licensure, assesses physical health needs, coordinates referrals to community health care providers, and ensures safe and effective medication coordination, including injectable medications, in conjunction with the individual's Psychiatrist or Psychiatric APRN. The nurse serves as primary case manager for an assigned group of individuals and promotes recovery, empowerment, self-determination, and shared decision-making for each FACT-IL participant. FACT-IL provides community-based behavioral health treatment and support to adults with serious mental illness using a multidisciplinary clinical case management model with medical services, wellness management, and recovery support. Services are designed to decrease crisis episodes, promote independent functioning, improve community tenure and involvement, and support successful transitions to community-based care. The nurse also provides limited nursing support to the Forensic Multidisciplinary Team (MFT), focused on coordination with the FACT-IL/MFT Combined Psychiatric APRN, pharmacy, and medication room operations, and consultation to MFT staff on nursing and medication room processes.
