About The Position

At Easterseals PORT Health (ESPH), the mission is to empower individuals and strengthen communities, particularly focusing on at-risk youth. The organization is seeking a full-time Wraparound Facilitator in the Goldsboro, NC area to join a caring, community-focused team. The role involves transforming lives by delivering services that empower youth and families to overcome challenges and build brighter futures. Easterseals PORT Health has been voted Raleigh’s Best Nonprofit Organization and Raleigh’s Best Mental Health Services two years in a row. As a QP Facilitator on the High-Fidelity Wraparound team, the individual will build and lead the Child and Family Team (CFT) to support youth in staying “at home, in school, and out of trouble.” This includes coordinating team activities, providing direct support and education to families and team members, and guiding families in accomplishing objectives outlined in the Family Plan using a strengths-based approach. The facilitator will also ensure accurate documentation in the UNCG database and embody the Wraparound philosophy of “Whatever it takes” to meet the unique needs of youth and families, driving meaningful, lasting success. The service area covers Duplin, Edgecombe, Lenoir, Sampson, Wayne, Wilson, and Greene counties. Requirements

  • Bachelor’s degree in Social Work, Psychology, or related Human Services field
  • Must meet the NC Qualified Professional requirements to include education and experience with the population served
  • A valid driver's license, current auto insurance and a reliable vehicle for work-related travel

Responsibilities

  • Build and lead the Child and Family Team (CFT) to support youth on a mission to stay “at home, in school, and out of trouble”
  • Coordinate team activities to achieve the goal of supporting youth
  • Provide direct support and education to families and team members
  • Guide the family in accomplishing objectives outlined in the Family Plan using a strengths-based approach
  • Ensure accurate documentation in the UNCG database
