We’re seeking a full-time Wraparound Facilitator in the Winston-Salem, NC area to become an essential part of a caring, community-focused team. Together, we transform lives by delivering services that empower youth and families to overcome challenges and build brighter futures. Your Role in Our Mission As a QP Facilitator on the High-Fidelity Wraparound team, you will build and lead the Child and Family Team (CFT) to support youth on a mission to stay “at home, in school, and out of trouble.” You will coordinate team activities to achieve this goal while providing direct support and education to families and team members. Using a strengths-based approach, you will guide the family in accomplishing objectives outlined in the Family Plan and ensure accurate documentation in the UNCG database. This full-time position offers a flexible schedule primarily business hours, but does include 3-7pm to accommodate the family’s schedules as needed. Requirements

  • Bachelor’s degree in Social Work, Psychology, or related Human Services field
  • Must meet the NC Qualified Professional requirements to include education and experience with the population served
  • A valid driver's license, current auto insurance and a reliable vehicle for work-related travel

Responsibilities
