About The Position

This position will be responsible for leading and coordinating the Extended Foster Care (EFC) staff in the support of services to eligible young adults who have transitioned from licensed foster care ages 18-21. These services include but are not limited to life skills training, community outreach, referrals and system navigation to ensure that the emotional, physical, behavioral and mental health needs of young adults are met. This position will ensure that appropriate living arrangements are established that will allow for the young adult’s personal development while providing necessary support, guidance, and continued education and employment.

Requirements

Education: Bachelor’s degree required; Master’s degree preferred.
Experience: At least three years’ experience coordinating projects, working with community partners and managing staff. Minimum three years’ experience working in the child welfare system work with high-risk youth. Experience working with youth and young adults preferred.
Licensure, Certification, and/or Registration: Valid driver’s license. Child Welfare Certification

Responsibilities

Develop, mentor, coach, and supervise assigned staff to promote client safety, well-being and a successful transition into adulthood in accordance with Florida Statute. Identify and analyze problem areas in order to improve quality of care. Monitor and assess the implementation of interventions being utilized by staff in the program.
Evaluate assigned staff competency in all job requirements and provide regular feedback.
Review all monthly progress reports as required to verify program compliance of participants. Review and submit weekly/monthly check requests.
Ensure all home visits and documentation are completed on time by staff.
Monitor and report on contractual outcomes related to the completion of Transition Plans, Case Plans, Judicial Reviews, EFC Initial Program Applications and Readmission Applications, Voluntary Placement Agreements, Shared Living Plans, Supervised Living Arrangement Assessments, Participant Budgets, Housing Partnership Agreements, and any other relevant program documents.
Conducts on-going risk management of all cases assigned to unit and implements plans to alleviate these risks.
Monitor and follow-up with staff regarding scheduled court hearings to ensure proper attendance/coverage, the outcome of court hearings and documentation in FSFN. Attend court hearings to provide support to staff and/or young adult as needed.
Coordinate and/or conduct client hands-on training for skills necessary to live independently. Training will consist of at a minimum budgeting, nutrition, family planning, education planning, job training, obtaining and maintaining housing and employment and ensure that service delivery is community based allowing for experiential learning opportunities for young adults.
Facilitate conferences/meetings in effort to prevent placement and/or program disruption, service interruption, or termination. Facilitate emergency staffings related to young adults encountering crisis situations.
Provide 24/7 on-call crisis support.
Advocate for greater access to affordable housing, public transportation, health, educational, and employment opportunities for participants.
Assist the Manager of Youth Well-Being & Independent Living with community activities and presentations.
