This position will be responsible for leading and coordinating the Extended Foster Care (EFC) staff in the support of services to eligible young adults who have transitioned from licensed foster care ages 18-21. These services include but are not limited to life skills training, community outreach, referrals and system navigation to ensure that the emotional, physical, behavioral and mental health needs of young adults are met. This position will ensure that appropriate living arrangements are established that will allow for the young adult’s personal development while providing necessary support, guidance, and continued education and employment.
