This role is responsible for providing music therapy services to patients.
Assess music therapy needs of the patient/family and provide music therapy services to the hospice patients and their families, as appropriate.
Consult with interdisciplinary team about patient referrals to assess needs and prioritize intervention.
Document all care provided on the patient’s clinical record in accordance with Hospice’s documentation requirements.
Educate hospice staff and community about the role of the Music Therapist and the use of music therapy interventions with hospice patients and their families.
Assist with facilitating support groups and special events upon request and ensure effective use of resources
Serve as an education resource to staff, volunteers, care partners and the community.
Support and promote the Hospice mission to lead in providing exceptional patient-centered end-of-life care and grief support.
Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
Demonstrates a knowledge and understanding of what to report to the CEO or Chief Compliance Officer when concerns of corporate compliance arise.
Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures.
Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines.
