Requirements
- Licensed Practical Nurse (LPN) license required
- BLS required
- Experience in placing peripheral IV's, accessing ports and working with PICC lines required
- Ability to use sound judgement and remain calm in a variety of situations
- Basic computer skills
- Proof of eligibility to work in the U.S .
Responsibilities
- Deliver exceptional care to the dedicated client in their home
- Help execute comprehensive personalized weekly schedules and care plans
- Provide skilled nursing services as prescripted under a client's plan of care that can be safely provided in the home, within your scope of practice
- Counsel clients and family in meeting nursing and related needs
- Meet all documentation requirements according to agency policy and proecedure
- Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated
- Administrative functions such as documentation of completion of tasks and report changes in the client’s physical condition, mental capability or behavior
