A Brief Overview Develops and implements age-and-ability appropriate physical activity programs and mind-body exercises practice of patients participating in the Weight and Wellness and Eating Disorders Program. Educates Patients and families on the role of exercise and activity in managing overall health and well-being. Works with the multidisciplinary team, making recommendations to optimize patient fitness and activity progress.

Requirements

  • Bachelor's Degree from an accredited college or university in exercise science, health promotion, fitness & wellness, physical education, health education or other health or related field Required
  • Current and valid Basic Life Support (BLS) through the American Heart Association Required
  • ACSM - Clinical Exercise Specialist (Eating Disorders only) within 1 Year Required
  • Knowledgeable in the areas of health, fitness, exercise, wellness and evidence-based community support programs
  • Strong interpersonal skills and ability to communicate clearly and effectively to varied audiences both verbally and in writing
  • Relies on experience and judgment to develop program plan, establish and accomplish goals
  • Ability to lead and direct the work of others
  • Excellent problem solving skills
  • Ability to motivate and educate diverse populations
  • Ability to be self-directed, working with minimal direction
  • Ability to work collaboratively with other team members

Nice To Haves

  • Experience working with school-aged children and adolescents in an educational or fitness setting Preferred

Responsibilities

  • Develops and implements age- and ability-appropriate physical activity programs and mind-body exercise practice for clients.
  • Identify and implement evidence-based group fitness programming for patients and families
  • Works with IT in the development and maintenance of group fitness scheduling template
  • Responsible for class instruction of group fitness classes, ensuring all classes are covered either by self or by fitness assistants/volunteers and that proper oversight/supervision are available for classes as appropriate
  • Participates in the selection and direct supervision of fitness assistants as appropriate
  • Responsible for selection and management of fitness volunteers
  • Works with the community to serve as expert resource for team and patients and to promote family friendly community activity offerings
  • Serves as an expert to the team regarding available family fitness offerings and resources within the community
  • Serves as the fitness and activity representative of the team, planning and presenting educational offerings and activities to patients, other providers and/or in community settings
  • Works with clients and families to establish individualized and realistic exercise/activity goals that will contribute to healthy lifestyles.
  • Implements and oversees the physical activity/exercise component of the program for each patient, modifying and revising as needed to meet the needs of the patient and family. Ensures that all participants are properly trained on the use of the program’s exercise equipment. Ensures the fitness program meets the needs of the client and may include elements such as physical fitness, cardiovascular training, mind-body exercise and weight training. Includes 1:1 training on proper body mechanics as appropriate
  • Schedules and conducts individual fitness consult visits per provider recommendations
  • Works with clients and families to establish individualized and realistic exercise/activity goals that will contribute to weight management and reduction in related co-morbidities
  • Identifies risk factors and designs specialized strength, conditioning and physical activity/exercise programs that are specific to the medical needs of clients.
  • Prepares a plan and supports patients and families between clinic visits as determined by the team
  • Assesses and re-assesses client status, progress and continued needs; modifies treatment plan as needed. Offers on-going guidance and support to clients and families in order to support goal accomplishment
  • Educates clients and families on the role of and importance of physical exercise/activity, providing guidance and instruction on incorporating new practices into daily life styles. Educates clients and family members on proper techniques for transitioning from program-based fitness program to a safe and effective long term home- & community -based activity plan
  • Communicates and collaborates with medical team to promote continuity of care for patients and to promote individual client and program success. Reports out regularly to the multidisciplinary team on patient fitness progress and recommendations
  • Completes all necessary documentation in a timely fashion
  • Provides high quality patient care that reflects appropriate clinical reasoning, evidence-based practice, and that optimizes patient’s improved quality of life
  • Works with medical director, multidisciplinary team and research department in conducting or assisting with pertinent research and/or data collection.
  • Manages fitness data collection including the 6 minute walk test or other fitness tests as appropriate
  • Monitors the effectiveness/efficiency of program to support the delivery of quality patient care, recommending and implementing changes as necessary.
  • Works with clinic and leadership to continuously evaluate process, identify problems, and propose/develop process improvement strategies
  • Develops or edits policies/protocols in relation to fitness component of the program
  • Regular attendance at work is an essential function of the job.
