About The Position

Engages at-risk patients, employees, eligible, beneficiaries and underserved residents that have been referred by providers, payers and community organizations to assist these individuals as part of a defined at-risk population group to initiate participation in Physical Activity(PA) Intervention Programs based on individual health status, demographics and economic considerations. Requirements

  • Bachelor's Degree in Exercise Science, Athletic Training or related field of study
  • No experience required. Two (2) years of wellness/fitness center, motivational interviewing, wellness education preferred.
  • Exercise is Medicine Credential thru the American College of Sports Medicine (EIM) - must be obtained within 3 months of hire date
  • Current National Commission for Certifying Agencies (NCCA) accredited exercise certification
  • Proficient computer skills (word processing, spreadsheets)
  • Mathematical skills

Responsibilities

  • Coordinates Exercise is Medicine Greenville (EIMG ) referrals and related information between the three partnering institutions that constitute EIMG: Prisma Health System, USCSOMG, YMCAs of Greenville, and Foothills YMCA
  • Serves as the clinical liaison to the Prisma Health System medical partners and practices to include care management validation for risk assessment.
  • Enrolls referred individuals into the web-based EIM Network based on their primary at-risk stratification and create a personalized PA Prescription utilizing assessments, profile tools, and motivational interviewing techniques to determine individual level of self-confidence and readiness to participate. Based on readiness, PA Prescription and ability to pay, assist each enrolled individual to select the appropriate PA Intervention Program and Place.
  • Transitions each enrolled individual to the care of the EIM Professional delivering their selected PA Intervention Program and Place. Follows up with each enrolled individual on agreed upon timeframe to promote initial participation in their selected PA Intervention Program.
  • Assists with Continuing Education related to EIMG.
  • Leads Facility Coordinator Update meetings, providing updates to teams and stakeholders. Manages meeting agendas and schedules. Reports to Exercise is Medicine Advisory Board at monthly meetings.
  • Coordinates updates to EIMG website.
  • On-boards new practices into the EIMG program.
  • Supports research and reporting efforts of program. Maintains reporting referral numbers to EIMG team, keeping all worksheets up-to-date.
  • Performs other duties as assigned
