The Executive Director of Care Management WFD will be responsible for developing and executing the strategic vision for care management across the WFD. This position will lead a care management strategy that enhances the consumer experience and effectively deploys Care Management resources. They will develop strategies supporting decreased variability in care management practices while working to embed evidence-based care transition practices. The executive director will partner with the SEO Clinical Outcomes, SEO Finance, and Corporate Exec Director of Care Management to develop strategies to reduce length of stay, reduce readmissions, improve STAR ratings for the division, and simplify daily workflows all while improving governance and accountability structures. The position will report directly to the SEO Clinical Outcomes and work collaboratively with the SEO Finance WFD, and Corporate Exec Director of CM, WFD Directors of CM, and members of the AdventHealth Clinical Team to align a vision for care management that supports the organizational strategic goals. Additionally, this role will have operational accountability for Post-Acute Networks, BPCI-A for WFD.
