Requirements

• Arizona Special Education Cross Categorical, Mild/Moderate, or LD Certificate K-12 OR Arizona Early Childhood, Elementary, Middle Grades OR Secondary Certificate WITH Mild-Moderate Special Education Endorsement, PreK-12 OR Arizona Special Education – Hearing Impaired, Visually Impaired, Early Childhood Special Education OR Moderate/Severe Disabilities WITH Mild-Moderate Special Education Endorsement, PreK-12
• Current Arizona IVP Fingerprint Clearance Card
• Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization.
