About The Position

The purpose of this job is to ensure that students with disabilities learn and achieve at their highest potential. The Transition Academy Teacher provides transition support to special education teachers and secondary students age 14 to 22 with IEPs. This role involves collaborating with students, parents, and other IEP team members to facilitate effective transition services and coordinating community-based work programs.

Requirements

  • Hold a valid state of Tennessee teaching certification in Special Education (Endorsements: 105 or 461)
  • If you hold a teaching license in another state, you must apply for a Tennessee teaching license.
  • Hold a bachelor's degree
  • Minimum of three (3) years of successful classroom teaching experience
  • Experience assisting students with disabilities in obtaining and/or maintaining employment
  • Knowledge of the transition process and adult transition agencies
  • Possess a valid Motor Vehicle Operator’s license and have daily access to a car
  • Ability to communicate effectively both verbally and in written form
  • Ability to exercise respect, patience, courtesy, tact, and good judgment when working with students with disabilities and their families
  • Ability to employ business technology tools (e-mail, Zoom, Microsoft Office Suite, etc.).
  • Ability to work collaboratively with students, parents, school personnel, agencies/ organizations and community business partners.
  • Demonstrate ability to effectively work with diverse populations
  • Good organizational skills
  • Good communication skills to facilitate teaching, assessment, meeting and data management responsibilities such as Easy IEP

Responsibilities

  • Provides leadership and coordination to the Transition Academy
  • Recruits new student interns for the program including: Marketing to local schools and agencies including Information sessions for families, WBL sites, schools and agencies, Determining recruitment timeline, Determining selection committee; prepares rubric information, Holding selection process with team, Sending letters to all applicants, Recognizing that student recruitment is an ongoing commitment
  • Develops IEP and transition goals for new students and creates “career plan” based on individual preferences and skills
  • Coordinates and/or provides travel training when public transportation is available
  • Assesses student skills and gathers baseline data
  • Coordinates Work Based Learning experiences by developing business and community partnerships
  • Assists in linking students to post-secondary education programs
  • Provides basic information about social security benefits, work-related expenses and Ticket-to-Work program
  • Plans and provides daily classroom instruction in employability skills
  • Models appropriate workplace behavior
  • Trains students in interview process
  • Communicates with parents on a regular basis
  • Performs skills training duties to assist with skills acquisition and problem solving
  • Plan Transition Academy events such as orientation, information nights/open house and graduation
  • Works with other team members to provide internal and external marketing for businesses and the community such as “Lunch and Learn” events, newsletter articles, website information, tours, community presentations, etc.
  • Completes necessary evaluations, reports and other documentation
  • Develops a portfolio for each student that documents each work site, skills acquired, evaluations, letters of recommendations, etc.
  • To identify a student’s strengths, interests, and abilities related to skill acquisition, job development and placement
  • To seek appropriate community job sites consistent with the students interests and skills
  • To determine and refer to appropriate community support services for training and successful employment
  • Ability to provide supervision and instruction for students with social, behavioral, communication, physical, self-help and cognitive disabilities
  • Knowledge of IDEIA, current state special education rules and regulations, and compliance issues relative to programming, assessment and procedural safeguards for students with disabilities
  • Obtain and maintain Work-Based Learning certification
  • Prepare students for potential participation in the Project SEARCH program
  • Assist the Project SEARCH team by recommending and participating in the student selection process
  • Participate in Skills Assessment Day
  • Other duties as assigned
  • May include providing general physical assistance to students with various disabilities. Tasks may involve, but are not limited to, assisting students with toileting/feeding/other self-help and behavioral needs.
  • Participate in training programs related to delivery of IEPs or curriculum
  • Maintain all required special education records, ensuring that they are complete, accurate and within all timelines.
  • Attends work and arrives to work on time.
  • Maintains confidentiality.
  • Adheres to the Teacher Code of Ethics as defined by Tennessee Law
