About The Position

At Brighton/Suncrest Hospice, our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients. This is achieved by allocating the resources to increase our staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Requirements

• Current Licensed Vocational Nurse (LVN) licensure in good standing in the state(s) of practice with one year of experience under license.
• Ability to work well within a team environment.
• Strong time management skills to manage visits and documentation.
• Ability to learn an electronic medical records system.
• Must have reliable transportation, a valid driver's license, and auto insurance.

Responsibilities

• Providing care and services for a group of patients in various settings including private homes, Skilled Nursing Facilities, and Assisted Living Facilities.
• Contributing as a member of the interdisciplinary team, providing expertise regarding psych-social assessments and interventions.
• Collaborating daily with a team of clinicians to ensure quality patient care.
• Managing visits, documentation, and meetings effectively.
