About The Position

The evening case manager performs duties relating to case management including, assisting clients in obtaining supportive services within community to meet client needs. facilitates individual and group services. ESSENTIAL FUNCTIONS Essential functions are duties which are essential to the position or the reason the position exists. The Evening Case Manager performs an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties:

Requirements

• High School Diploma or equivalent.
• Knowledge of Substance Use Disorder and Community Psychiatric Supportive Treatment.
• Valid Ohio Driver's License and ability to maintain insurability preferred.
• Maintain CPR/First Aid treatment preferred.
• Maintains applicable agency trainings.
• Experience with 12-Step Recovery Program preferred.

Responsibilities

• Assesses needs of clients to link to community resources to meet client needs.
• Assists in obtaining supportive services to aid the client in their personal goals in recovery through the services of the agency or through referral to other agencies or programs. Assist clients in accessing community resources and provides supportive services.
• Provides group and individual educational services to improve skills that support personal recovery efforts.
• Provides written documentation of client activities to maintain compliance with clinical requirements and provides documentation for billing of services.
• Provides education on daily living skills within the supportive housing environment.
• Required to attend weekly supervision and Case Management Training.
• Performs other duties as assigned by the Program Coordinator/Director.
