About The Position

Engage target consumers into ongoing treatment, coordinating care for the whole person by addressing behavioral health, physical health and basic needs, reducing the need for future emergency visits, hospitalizations and reducing hospital stays unnecessarily extended due to non-clinical factors. You will be working in the community which includes consumer’s homes, clinics, and other places around the community.

Requirements

  • Requires a bachelor’s degree in a human services field.
  • Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
  • Completion of New Hire Orientation at the beginning of employment.
  • All training requirements including Relias at the beginning of employment and annually thereafter.
  • Current driver’s license, acceptable driving record and current auto insurance.

Responsibilities

  • Responds to inquiries for mental health, domestic violence, and/or addictive disorder services and refers to appropriate system programs or outside agency(s), and community resources as appropriate.
  • Help locate resources, apply for these services, set appointments, provide accurate information, advocate, and follow up when necessary.
  • Maintain a system of records and reports necessary for documentation of referrals at specific time intervals.
  • Provide interim triage contact with consumers, and assist with connecting to behavioral and crisis intervention services.
  • Deliver supportive services to assigned consumers until they have established care with providers who will manage their on-going treatment needs.
  • Provide and/or coordinate follow-up to all consumers for whom he/she has been responsible, including follow up for missed appointments, and conducting outreach and engagement as needed.
  • Collaborate as part of a team which may include hospitals and medical clinic health professionals or other employees of a designated community partner.
  • Attend individual supervision and team meetings as scheduled by the ERE Supervisor.
  • Follows all policies and procedures as outlined by the system, the Missouri Behavioral Health Council, and the Department of Mental Health.
