Requirements

• Graduate of an accredited nursing program with a current RN license in OK.
• 1-2 years of recent nursing experience (home health experience preferred).
• Strong communication, critical thinking, and interpersonal abilities.
• Current CPR/First Aid certification from the American Heart Association.
• Capable of occasionally exerting 50-100 pounds of force.
• Valid driver's license, reliable vehicle, and proof of insurance.

• Familiarity with Medicare home health.

Responsibilities

• Coordinate and implement individualized care plans, revising as necessary.
• Educate patients and their families on disease processes, self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Document clinical notes and patient progress to meet established goals.
• Collaborate with the interdisciplinary team to prioritize and deliver care effectively.
• Supervise LPNs and Home Health aides, ensuring quality standards are met.
• Provide on-call coverage as part of a rotation schedule.
