Requirements

  • Current CPR/First Aid certification from the American Heart Association.
  • Capable of occasionally exerting 50-100 pounds of force.
  • Valid driver's license, reliable vehicle, and proof of insurance.

Responsibilities

  • Coordinate and implement individualized care plans, revising as necessary.
  • Educate patients and their families on disease processes, self-care.
  • Initiate preventive and rehabilitative nursing procedures.
  • Document clinical notes and patient progress to meet established goals.
  • Collaborate with the interdisciplinary team to prioritize and deliver care effectively.
  • Supervise LPNs and Home Health aides, ensuring quality standards are met.
  • Provide on-call coverage as part of a rotation schedule.
