About The Position

The Environmental Modifications Care Management Associate (EM CMA) plays a key role in supporting Long‑Term Services and Supports (LTSS) members by coordinating home modification projects that improve safety, accessibility, and quality of life. In this role, you’ll serve as the primary point of coordination for Environmental Modification (EM) requests—from initial referral through project completion. You’ll partner with Care Managers, contractors, Medical Directors, and members to ensure projects are delivered efficiently, compliantly, and with a strong focus on member satisfaction. This position is ideal for someone who enjoys project coordination, vendor collaboration, and mission‑driven work.

Requirements

  • Experience in Medicaid, LTSS, waiver programs, or healthcare administrative support
  • Experience with authorizations, utilization management, or vendor coordination
  • Ability to review contractor bids, pricing, and scopes of work
  • Strong organizational, time‑management, and communication skills
  • High School Diploma or equivalent

Nice To Haves

  • Experience with Environmental Modifications or HCBS programs
  • Knowledge of state Medicaid policies and medical necessity criteria
  • Experience working with contractors or vendors
  • Proficiency with utilization management systems and Microsoft Office
  • Associate degree

Responsibilities
