About The Position

Holly Hill Healthcare Center, located in Towson, MD, and a proud subsidiary of CommuniCare Health Services, is seeking a detail-oriented Registered Nurse (RN) to join our team as an Entry-Level MDS Coordinator. This role is designed for an RN interested in developing expertise in MDS assessments, clinical documentation, quality measures, and reimbursement. No prior MDS-specific experience is required, but at least 2 years of prior direct care experience as an RN is required, with a preference for experience in long-term care, post-acute, or skilled nursing environments. Comprehensive MDS training is provided, and certification is required within 12 months of start date.

Requirements

RN license required

MDS training supported; certification required within 12 months of start date
Strong organizational, analytical, and documentation skills
Proficiency with electronic health records preferred

Responsibilities

Coordinate and complete MDS assessments in accordance with CMS and regulatory requirements
Ensure accurate, timely, and compliant clinical documentation
Collaborate with the interdisciplinary team to support care planning and quality outcomes
Track assessment schedules and submission deadlines
Support quality improvement initiatives and survey readiness activities
