The Enrollment RN plays an essential role in helping seniors access the comprehensive care they deserve through our PACE program. As part of the WelbeHealth Marketing, Outreach, and Enrollment (MOE) team, the Enrollment RN partners closely with Physicians, Social Workers, Outreach colleagues, and Caregivers to conduct thoughtful, timely assessments of prospective participants. Through collaboration and clinical expertise, the Enrollment RN helps ensure every individual receives a smooth and supportive path to enrollment. This role is different because the Enrollment RN at WelbeHealth: Plays a key role in launching WelbeHealth’s first center in New Jersey, helping to build and shape our new market Uses nursing expertise to guide seniors and their families through the enrollment process, directing access to life-changing care through PACE Collaborates with a diverse range of Clinicians and members of MOE team We care about our team members. On the day-to-day, you will: Conduct initial nursing assessments (LOC), including but not limited to nursing evaluations, functional assessments, home safety evaluations, and calculation of recommended home care hours, in prospective WelbeHealth participants’ homes to determine if they meet PACE eligibility requirements Perform all required home visits to complete in-person assessments, ensuring accurate evaluation of participants’ home environments and care needs Promptly prepare documentation from LOC assessments and submit to state for review, responding to questions to expedite the review process for participants. Manage administrative responsibilities and communication associated with the state and the central ERD Communicate cross-functionally with central ERD and IDT, and track completion of various assessments as well as ensure timely LOC submission Attend center events as directed by supervisor or MOE Director
Requirements
- Graduate of an accredited school of nursing (BSN preferred)
- Unencumbered RN License
- Training and passing of exam/certification as a Marketing Representative within thirty (30) days of hire
- Completion of required state certification for LOC assessments
- Valid driver's license and reliable means of transportation
