About The Position
Guthrie Lourdes is seeking a dedicated and passionate Nurse Practitioner to assist in diabetes management and general endocrine care of our patients in the outpatient practice setting of Guthrie Binghamton Riverside Drive-Endocrinology. As a Guthrie Nurse Practitioner, you play an important role in caring for our patients while acting as a patient advocate and collaborating with physicians and advanced practice providers, as well as Guthrie Specialists throughout the strong referral Network. This position involves assessing, diagnosing, and managing patients throughout the continuum of care, which includes conducting comprehensive assessments of patients with various endocrine disorders including diabetes and thyroid conditions. You will be responsible for developing and implementing individualized treatment plans, considering patients' unique needs and preferences, and performing regular follow-ups to assess treatment effectiveness and adjusting plans as needed. In addition to clinical responsibilities, you will educate patients on diabetes self-management, lifestyle modifications, and the importance of adhering to prescribed treatment plans. Providing clear and concise information on endocrine conditions is crucial, ensuring patients have a thorough understanding of their health and treatment options. Collaboration with physicians, nurses, and other healthcare professionals is essential to ensure coordinated and patient-centered care. You will also maintain accurate and up-to-date electronic health records, documenting patient assessments, interventions, and outcomes while ensuring compliance with regulatory requirements and organizational standards.
