Under the supervision of the IDD Supported Employment Program Administrator develop competitive integrated employment opportunities, provide on the job skills training to individuals with significant disabilities. Observe the individual to identify and solve potential problems related to the individual’s employment success while gradually reducing the time spent with the individual at the job site and maintaining communication with employer, family and Texas Workforce Solutions Vocational Rehabilitation Counselors. Manage a caseload of 15-25 individuals ensuring that services are provided according to Texas Workforce Solutions Vocational Rehabilitation Provider Standards. Must work well with community partners and be an active part of the Supported Employment Team. Irregular work hours may include evenings, weekends and occasional holidays based on the needs of the caseload. This position will require travel from site to site throughout the community based on the needs of the caseload. Additionally: Complete 90% of documentation into electronic health record within the required timeframe of 48 business hours. Complete all Career Planning Assessments and submit for billing within designated time frame. Develop a minimum of 13 employment opportunities annually that meet the individual’s employment plan. Engage with individuals receiving service, provide exceptional customer service, and support for a minimum of 60 hours per month. Track billing dates, complete and submit benchmark billing documentation within 5 business days of the date of service. Provide monthly email updates to Texas Workforce Solutions Vocational Rehabilitation Counselors and Rehabilitation Assistants. Assist in additional Supported Employment programs as needed for coverage.
