About The Position

Position assesses and provides psychiatric evaluation for patients seen for mental health, substance use, and medical social work needs, and provides clinical therapeutic services to include therapeutic modalities, crisis intervention, and case management services to patients and families.

Requirements

  • Master's degree in Social Work or related field required
  • Active license in the State of Idaho as a LCSW or commitment to obtaining Idaho LCSW within 6 months of hire required
  • Clinical licensing preferred
  • Minimum 2 years of experience providing service in a mental health setting, such as community mental health, required
  • Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.). Compliance with accepted professional standards and practices
  • Basic knowledge of pharmacology for common mental health disorders that within scope of practice

Responsibilities

  • Provides general social work services linking patients and families to appropriate referrals and/or assisting them in meeting their needs
  • Screens and assesses patients for social determinants and connects, links, and refers patients to appropriate resources
  • Completes biopsychosocial, psychiatric, and Columbia Suicide Severity Rating Scale as a part of comprehensive assessment process for both psychiatric and/or medical social work needs
  • Conducts strengths based, patient-centered assessments, providing evidenced based interventions
  • Supports the mental and physical health care of patients on an assigned daily caseload, coordinates care with patient's primary care doctor, and consults with psychiatrist and emergency department physicians
  • Systematically tracks plan of care and monitor patient disposition
  • Provides social work intervention for placement, home health, hospice, palliative care, and other discharge needs
  • Provides consultation for safety and mandating reporting to vulnerable populations
  • Understands insurance payer requirements and processes for resources and disposition
  • Completes clinical documentation within required timelines
  • Develops and complete safety plans and provides follow up
  • Identifies high utilization and completion of EDIE
  • Participates in and uses clinical supervision and consultation to continuously examine professional roles and boundaries, engage in ongoing self-correction, and to insure that practice is congruent with NASW code of ethics and values
  • Performs other related duties as assigned
  • Familiar with standard concepts, practices, and procedures within the field
  • Regular and predictable attendance is an essential job function
  • Competent to meet age specific needs of the unit assigned
