About The Position
This position supports Emergency Department operations through care coordination, discharge planning, and utilization management to improve patient throughput and transitions of care.
Requirements
- NY RN License (active)
- 5+ years clinical RN experience
- Experience in discharge planning and utilization review
- ASN/ADN required
- BLS required
Nice To Haves
- Bachelor’s or Master’s degree
- Case Management background
Responsibilities
- Manage care coordination for ED patients
- Conduct utilization reviews and discharge assessments
- Partner with clinical teams to reduce length of stay
- Arrange post-discharge services and resources
- Ensure documentation meets regulatory guidelines
