Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Requires prioritization and critical thinking skills, a decisive judgement, and ability to work with minimal supervision in a fast-paced environment. Comply with triage, treatment, evaluation, and documentation guidelines. Uses evidence-based practices and competence to collaborate with the healthcare team to provide quality outcomes and coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Identifies barriers to effective communication based on psychosocial, literacy, financial, and cultural considerations. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstance. Effectively delegates patient care to ancillary personnel. May precept student nurses and new hires. Practice according to professional and ethical standards as noted in the ANA Code of Ethics and Colorado State Nurse Practice Act. Demonstrates caring attitudes and behaviors to develop therapeutic relationships and engage in patient and family teaching as appropriate Perform other duties as assigned. Strives to uphold regulatory requirements to ensure continual compliance with departmental, hospital, state, and federal regulations and policies.
