The ED pharmacist shall optimize medication use through participation in direct patient care rounds, medication order review, medication therapy monitoring, participation in procedures that utilize high-risk medications, resuscitation, medication procurement and preparation, provision of medication information and documentation of associated interventions.
Requirements
- Graduate of an ACPE-accredited School of Pharmacy
- Currently licensed to practice pharmacy in the state of Massachusetts
- Health system experience required
- BLS required
Nice To Haves
- Pharm D preferred
- Emergency Medicine experience preferred
- ACLS and PALS preferred
Responsibilities
- Medication order review
- Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols.
- Resusitation.
- Education and discharge planning
- Facilitate procurement of medications on weekends when main pharmacy is closed
