About The Position

The Community Healing Initiative (CHI) is a partnership with POIC + RAHS, and Multnomah County's Department of Community Justice is in search of a Care Manager to work with African American adult males, ages 17-25, who are gang impacted, have committed high level misdemeanors and/or lower level offenses, and are at high risk of further penetration into the criminal justice system. POIC + RAHS is seeking a highly enthusiastic, experienced, self-disciplined, and motivated Care Manager to provide intensive case management, assertive engagement, and crisis intervention to a caseload of adult males to reduce further penetration into the criminal justice system and incarceration. The Care Manager will work as part of the CHI Elevate for Black Men team to develop, implement, and evaluate the program.

Requirements

  • A passion for POIC + RAHS mission and serving at-risk young adults and their families.
  • Ability to establish and maintain effective working relationships with African American young adult men and their families.
  • Excellent problem-solving skills, highly organized, and can work well under high stress
  • Experience working with the criminal justice system and families impacted by gangs
  • Ability to maintain a high level of accountability, documentation, and community engagement/collaboration.
  • Bachelor’s degree required or equivalent and 3+ years demonstrated case management experience.
  • Experience working with communities of color, in particular African American young adult men and their families.
  • Experience and to work with people across the life span from children to older adults
  • Knowledge of and the ability to utilize community resources available for diagnosis, treatment, supervision, and support of young adults and their families.
  • Skill to assess risk factors, protective factors, personal and evaluation needs to develop appropriate individual interventions or case plans
  • Working knowledge of the symptoms of physical and mental health disorders, substance abuse, behavior and adjustment problems of juvenile offenders, and limitations or disabilities in children, adolescents, and adults.
  • Ability to facilitate and teach skill building groups to adult men
  • Flexible work schedule: will include weekend and evening work. Must be able to facilitate weekly evening groups.
  • Valid driver's license, reliable transportation and automobile insurance meeting minimum requirements as defined by the agency.
  • Successful completion of initial and on-going employment screens, including, but not limited to criminal background, sex offender, drug, abuse and neglect and vehicle record as required.

Responsibilities

  • Assess and implement strength-based individual success plans built on culturally specific and evidenced based practices to help participants develop and reach goals
  • Facilitate the “ Black Male Success ” curriculum to clients. The “Black Male Success “curriculum consists of workshops focused on developing skills, accessing resources, building relationships and overcoming barriers. Workshops will focus on home finance, health, employment, education, legal issues, and relationships.
  • Work collaboratively with other professionals in the field including schools, community based organizations, Mental Health/ Alcohol and Drug organizations, Child Welfare, Department of Community Justice, and Oregon Youth Authority
  • Provided day-to-day activities that may involve social casework, intake, assessment, assertive engagement, and skill development through engaging youth and families in activities through: Education/Employment, Home/Family, Health, Pro-Social Activities, Spirituality, and Community
  • Participate, develop and coordinate pro-social activities for adults and their families
  • Adhere to documentation requirements as indicated in CHI Program policies, including but not limited to accurate data entry into Service Point, case files, service plans, weekly/monthly reports, and case notes.
  • Respond to all crises and risk incidents within the required time frames per Portland OIC policies.
  • Other duties as assigned.
