About The Position
The Population Health Advisor at Blue Cross and Blue Shield of Louisiana is responsible for assisting medical groups and affiliate practices in implementing best clinical practices aimed at achieving better health outcomes, lower costs, improved patient experience, and enhanced clinician experience. This role focuses on maximizing the use of data to facilitate population health strategies and excel in value-based models, partnering with practices to redesign processes and workflows to improve health outcomes. Success is measured by reductions in total cost of care and improvements in quality and efficiency metrics.
