About The Position

The ED (Emergency Department) Nurse Navigator functions on the multidisciplinary team as an advocate, educator, and counselor for ED patients. Coordinates patient care and patient experience through follow-up with ED patients regarding outstanding diagnostic testing, patient feedback and patient callbacks. Serves as a liaison throughout the facility and in the community regarding services provided for the ED. Effectively collaborates with the patient, family, and members of the emergency department healthcare team. Complies with all existing policies and protocols related to patient care. Serves as an ED resource to facilitate communication across all aspects of the patient care experience. Within the reasonable realms of availability and accommodation responsibility addresses any issues with the patient and/or family they may be experiencing during or after the visit in the Emergency Department. Supports patient discharge through instruction and/or assistance with post ED care. Makes Follow-up calls to patients post discharge and offers navigation of care and community resources to prevent unnecessary patient revisits to the ED. Communicates and collaborates with other departments around care coordination. Provides teaching to patient and/or family.

Requirements

  • Current and unencumbered licensure as a Registered Nurse (RN) through the NCBON.
  • A minimum of 2 years working as an Emergency Department Nurse in a similar organization/department.
  • ACLS at hire or completed prior to the end of orientation.
  • PALS at or within three (3) months of hire.
  • NVCI/CPI Training (Non-Violent Crisis Intervention) initial certification within six (6) months of hire, refresher completed annually.
