This job is used to reduce the use of emergency departments for non-emergent reasons. This job will achieve streamlined patient care transitions and redirection to appropriate levels of care utilizing hospital and community resources to effectively educate and empower patients and their families. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Conducts Initial Evaluation and Engage Patient/Family.
  • Identifies Patient Needs and Connect Patients to Appropriate Resources.
  • Utilizes Behavior Change Strategies to Educate Patients and Families.
  • Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
  • Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
  • Education Required - Associate's Degree.
  • Preferred - Bachelor's Degree.
  • Work Experience Required - 1 year in a Clinical or Social Services setting working directly with patients.
  • Preferred - Experience in Case Management, Community based organizations, or Health Care organizations working directly with patients in a remote setting.
  • A Clinical License as a Medical Assistant, EMT, RN, LPN, Pharmacist, Social Worker, or other clinical discipline will be accepted in lieu of an Associate's Degree.
  • Must have computer skills and dexterity required for data entry and retrieval of information.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally.
  • Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
  • Strong interpersonal skills.
  • Strong oral and written communication skills.
  • Proven leadership skills, ability to think critically, problem-solve, work independently, self-initiate, multi-task, and work effectively in a team environment.
  • Highly organized and detail-oriented.
