About The Position

Under broad supervision of the Population Health Director, incumbent oversees Partnership Health Plan's Enhanced Care Management (ECM) Program, Care coordination team and other Anderson Valley Health Center Projects. Including but not limited to QI, grant specific projects, Public Health efforts, MOU agreements or projects with other community or healthcare agencies. Duties include case management, outreach for patients specific to these projects, and supervision of two Community Health Workers. ECM Program Coordinator will be responsible for all monthly reporting to Partnership Health Plan, reviewing and approving documentation and billing for all ECM encounters, attend ECM trainings, maintaining an up-to-date list of community resources, and lead team huddles and case conferencing. As a Community Health Worker (CHW), you will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports AVHC providers and teams through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. CHW provides social support and informal counseling, and advocates for individuals and community health needs. This position requires a bachelor's degree. Must be bilingual in English and Spanish.

Requirements

  • Bachelor's degree
  • 2 years of case management and patient care experience
  • Basic computer skills
  • Bilingual (English and Spanish)

  • 1 year of supervising at least two employees
  • Electronic Medical Record experience
