About The Position

Helpline Youth Counseling (HYC), a nonprofit agency serving Southeast Los Angeles County since 1971, provides therapeutic counseling, education, advocacy, and community resource connections to support at-risk children, youth, and families. HYC is seeking a compassionate and dedicated Enhanced Care Management (ECM) Lead Case Manager. This position serves as the lead care manager for Medi-Cal members with complex needs, coordinating health, behavioral, dental, and social services in collaboration with providers, community supports, and other entities. The ECM Case Manager’s goal is to improve health outcomes, reduce health disparities, and enhance continuity of care for high-need Medi-Cal enrollees, including those with chronic health conditions, behavioral health needs, substance use disorders, or who are experiencing homelessness or frequent hospitalizations.

Requirements

  • Bachelor’s degree in Behavioral Sciences or related field.
  • Minimum one (1) year of experience in community-based services.
  • U.S. citizenship or permanent residency.
  • Ability to pass criminal background clearance and TB screening.
  • Valid CA driver’s license, insurance, and reliable transportation.
  • Flexibility to work evenings and weekends.
  • Strong communication, organizational, and documentation skills.

  • Experience working with diverse populations in Southeast Los Angeles County.
  • Knowledge of Medi-Cal programs, managed care, and community-based services.
  • Prior experience with case management, care coordination, or similar roles.

Responsibilities

  • Conduct comprehensive health and psychosocial needs assessments.
  • Develop individualized, person-centered care plans with measurable goals.
  • Coordinate medical, behavioral, dental, long-term services and supports (LTSS), and community resources.
  • Facilitate seamless transitions of care, including hospital discharges.
  • Actively outreach and engage members through in-person visits, calls, texts, and community outreach.
  • Provide reminders, advocacy, and support to help members access and follow through with care.
  • Identify and address social determinants of health (SDOH) such as housing, transportation, food insecurity, or medication access.
  • Advocate on behalf of members with healthcare providers, hospitals, and service agencies.
  • Ensure culturally and linguistically appropriate communication and services.
  • Connect members with supportive resources such as housing, employment training, food programs, and social services.
  • Partner with managed care plans, hospitals, behavioral health providers, and community organizations to coordinate care.
  • Maintain accurate and timely documentation of assessments, care plans, member encounters, and progress.
  • Participate in trainings, clinical supervision, and quality assurance activities.
