The ECM Clinical Case Manager is responsible for the clinical care coordination of ECM Program clients, focusing on identifying, organizing, coordinating, and monitoring the services needed by recipients. This role primarily involves fieldwork to assist clients in accessing necessary services and community resources, ensuring a seamless experience for members.

  • Coordinate with individuals and entities to ensure a seamless experience for the Member and avoid duplication of services.
  • Engage eligible members and complete a bio-psycho-social assessment and a Care Plan.
  • Oversee the implementation of the Care Plan.
  • Develop effective and professional relationships with property owners/managers and housing providers.
  • Build trusting relationships with clients to promote their independence and long-term housing stability.
  • Facilitate linkage to mental health, substance abuse, and medical services, providing transportation when needed.
  • Communicate with mental health, substance abuse, and medical providers to ensure continuity of care.
  • Connect Members to other social services and supports they may need, including transportation.
  • Advocate on behalf of Members with healthcare professionals.
  • Utilize motivational interviewing, trauma-informed care, and harm-reduction approaches.
  • Coordinate with hospital staff on discharge plans.
  • Accompany members to office visits as needed and according to MCP guidelines.
  • Monitor treatment adherence, including medication.
  • Conduct face-to-face visits with Members as needed.
  • Maintain proper charting, progress notes, and case records for each enrolled Member.
  • Track interventions and outcomes.
  • Handle complaints from clients, families, or friends.
  • Report all signs of abuse or neglect.
  • Participate in education, training, and quality improvement activities.
  • Assist the Agency in maintaining compliance with Federal, State, Local, and HIPAA Regulations or Joint Commission Standards.
  • Establish and maintain good relationships with all Health Plans and other vendors.
  • Perform other duties as assigned.
  • Minimum of two years' experience working with homeless and/or low and mixed-income populations, as well as substance abuse and severe mental health issues.
  • Bachelor's Degree in a Health Care related field preferred.
  • Knowledge of community and housing resources and government benefits/welfare system.
  • Good verbal and written communication skills.
  • Ability to make sound clinical judgments regarding client care.
  • Licensed driver with an insured automobile in good working order, in accordance with state and/or organizational requirements.
  • Current CPR certification.
  • Current and satisfactory report on pre-employment physical examination including TB Screening Test or chest X-ray as required by Agency policies and procedures.
  • Must be free from signs of infection and illness.
