About The Position

The ECM Care Manager at Dignity Adult Day Health Care Center is responsible for providing case management services to high-utilizer populations, including adults with serious mental illness and substance use disorders. This part-time role involves outreach, health assessments, and care coordination to ensure members receive the necessary support and resources for their health and well-being.

Requirements

• Bachelor's Degree in Social Work or a related field.
• 1 year of experience in a related field.
• Understanding of strength-based services and core medical/mental health conditions.
• Familiarity with social determinants of health and care management.
• Ability to work with high utilizers and adults with serious mental illness or substance use disorders.
• Knowledge of HIPAA regulations and member confidentiality.

Responsibilities

• Report directly to the ECM Director.
• Complete member outreach activities, including locating members and educating them on ECM program benefits.
• Complete basic enrollment forms to initiate member receipt of ECM programs.
• Obtain and document verbal or written consent for care management purposes.
• Work in the field, including members' homes and community-based care settings.
• Complete comprehensive health assessments and home assessments as needed.
• Integrate motivational interviewing throughout all interactions with members.
• Develop resources, community networks, and services to meet members' needs identified in the Individual Plan of Care.
• Help members transition safely between different levels of care and execute needed care coordination.
