Job Summary: The ECM Assistant Manager supports the Site Manager in directing, organizing, and managing Serene Health's Enhanced Care Management Program (ECM). This role assists in coordinating the full range of physical health, behavioral health, and community-based long-term services and support needed by eligible beneficiaries while developing leadership skills and program expertise. Requirements

  • Bachelor’s degree from accredited college or university or equivalent experience required.
  • Minimum of 4 years of experience in a healthcare setting
  • Minimum of 2 years of supervisory or leadership experience
  • Experience working with multidisciplinary care teams
  • Understanding of social determinants of health and impact on patient well-being.
  • Knowledge of care coordination principles and best practices
  • Culturally competent and able to work with diverse populations.
  • Strong proficiency with Microsoft Office applications (Word, Excel, Outlook, etc).
  • Excellent verbal, written, and interpersonal communication skills.
  • Strong problem-solving and critical thinking abilities.
  • Demonstrated leadership and team management capabilities.
  • Ability to communicate effectively at all organizational levels.
  • Willingness to travel to different locations for training, meetings, and site visits.
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 10 pounds at times.
  • Able to work in various office environments.

Responsibilities

  • Provides direct supervision to Care Managers/Supervisors and support staff as assigned.
  • Provide direct supervision to company drivers and oversee driver logistics/operations and coordinate the maintenance and logistical requirements of the Serene Health vehicles.
  • Assists in leading multidisciplinary teams to provide seamless care coordination for high-need, high-risk populations.
  • Assists in developing, maintaining, and training staff on protocols, policies, and procedures.
  • Helps plan and facilitate ECM staff meetings, training sessions, and professional development activities.
  • Supports performance management processes under Site Manager direction.
  • Support Manager in mentoring site supervisors and providing role guidance as needed.
  • Support Manager with implementation strategies to boost retention and morale.
  • Acts as Site Manager in their absence.
  • Support development of program goals, staffing patterns, and work organization.
  • Supports liaison activities with other Serene Health departments and external referral resources.
  • Helps maintain professional relationships with Health Plan partners.
  • Assists in implementing systems of care coordination to enhance patient outcomes.
  • Monitors care coordination activities and provide guidance to team members.
  • Assists in ensuring Health Action Plans (HAPs) are created and maintained for all ECM patients.
  • Reviews patient progress toward HAP goals and escalates concerns as needed.
  • Supports quality assurance activities for care coordination processes.
  • Assist and support program supervisors with role processes and implementations.
  • Supports compliance monitoring to ensure adherence to ECM rules and regulations.
  • Extracts and analyzes data from various systems to support program evaluation.
  • Assists in gathering productivity and quality metrics from ECM systems.
  • Prepares reports and management information for leadership review.
  • Supports data-driven decision making for program improvements.
  • Upkeep program spreadsheet utilization.
  • Participates in recruitment and onboarding of new team members.
  • Participates in special projects and initiatives as needed.
  • Supports emergency coverage and crisis management when required.
  • Performs other duties as assigned Site Manager or ECM Operations Manager.
