Under the direction of the ECI Program Director, the Service Coordinator has the necessary authority to plan and provide services to eligible individuals of the ECI Program and travel within a 10 county area. Responsibilities include but are not limited to: Accept referrals for intervention services and conduct initial and subsequent enrollment activities in home and community settings. Assist parents of eligible children in identifying and gaining access to Early Intervention Services and other services identified in the Individual Family Service Plan (IFSP). Coordinate the provision of Early Intervention Services, as well as other services the child needs. Facilitate the timely delivery of available services. Continuously seek appropriate services and situations necessary to benefit the development of each child being served for the duration of the child’s eligibility. The ECI Service Coordinator must be able to lift, hold and play with young children and move in and out of sitting positions on the floor, as well as sit at a desk. Must also be able to operate a motor vehicle. The job requires extensive travel for service delivery and flexible hours and schedule. Telehealth practices may be employed as a strategy for service provision. HFC incorporates principles of trauma-informed care, which includes cultural sensitivity, in all areas of service delivery. Reassignment is a condition of employment that may occur at the employer’s discretion. Work location and hours could change based on program needs.
