About The Position

The Early Intervention Specialist (EIS) purpose is linkage and re-engagement to care for those Persons Living with HIV/AIDS (PLWHA). Identifying and addressing barriers to participating in adherence to ongoing HIV outpatient/ambulatory medical care. In addition, EIS is to ensure that people who test positive receive necessary HIV-related services as early as possible to interrupt or delay the progression of HIV disease and move a client toward self-management

Requirements

  • Education: Minimum of a high school diploma.

  • Experience: Experience working with HIV positive patients preferred.

Responsibilities

  • Provide counseling for HIV patients with respect to HIV/AIDS.
  • Provide referrals and link patients to needed medical and/or support services from multiple points of entry.
  • Provide HIV education, including risk prevention and adherence counseling.
  • Perform outreach activities for education and community engagement relating to HIV awareness.
  • Perform HIV testing in conjunction with pre/posttest counseling, as needed to make the unaware aware of their status.
  • Work with Medical Case Managers to identify out of care patients, or those at risk for becoming so.
  • Attends local and state planning meetings and training programs as deemed appropriate by supervisor. Attends monthly case management committee meeting.
  • Perform home visits to newly diagnosed patients, out of care patients, and/or patients with adherence issues.
  • Works with Medical Case Managers in assisting patients navigate the Ryan White eligibility process and Patient Assistance Programs, as needed.
  • Report data and encounters on patients in the Care Ware system.
  • May perform other duties as required.
