The Early Intervention Service Coordinator will work in partnership with the Stark County Board of Developmental Disabilities to provide Service Coordination for Early Intervention services for eligible children and families. The Service Coordinator's duties include providing service coordination to assist families in gaining access to early intervention services, ensuring parents' rights are upheld, participating in Stark County Early Intervention Teams, coordinating evaluations and assessments, and facilitating the development and implementation of Individualized Family Service Plans (IFSPs). The role also involves monitoring the delivery of early intervention services, facilitating transition plans from early intervention to preschool services, and advocating for children and families.

  • Be the single point of program contact for service coordination activities.
  • Assure parents are informed of their rights and obtain consent.
  • Participate as a member of Stark County's Early Intervention Team.
  • Ensure developmental screenings are completed and referrals for evaluations are made as needed.
  • Coordinate the child's evaluation, assessment, and family assessment within a 45-day timeline.
  • Develop and implement initial, annual, and periodic reviews of the IFSP.
  • Monitor the delivery of early intervention services to ensure they start within 30 days of a valid IFSP.
  • Complete the child outcome summary.
  • Facilitate the development of family outcomes.
  • Assist parents in establishing communication with a medical/health home.
  • Identify specialized services and providers.
  • Provide families with choices regarding service provider options.
  • Inform families about advocacy services.
  • Coordinate with medical and health providers.
  • Facilitate transitions to other programs and services.
  • Develop transition plans from early intervention to special education preschool or other services.
  • Maintain appropriate documentation for client records according to local rules and policies.
  • Communicate effectively and professionally with families, schools, and professionals.
  • Contribute to team efforts by integrating individual responsibilities.
  • Commit to continuous learning and self-improvement in early intervention and early childhood developments.
  • Minimum of a Bachelor's degree in Social Work, Education with focus on Early Childhood, Nursing, or a related field.
  • Licensure Required: LSW, EIS/DS, RN, ECE-Professional Teaching License in Early Childhood Education, OT, PT, SLP, or licensed in vision impairment intervention.
  • At least two years' experience working with families.
  • At least two years' experience working with children ages birth to three.
  • Experience using child development evaluation and assessment tools.
  • Knowledge of community resources.
  • Valid State of Ohio driver's license and proof of insurance.
  • Home visiting experience.
  • At least one year of previous service coordination experience.
  • Data entry experience.
  • Additional or alternative qualifications as deemed appropriate.
  • Beginning salary of $40,290.
