New York Therapy Placement Services, Inc. (NYTPS) is seeking an Early Intervention Ongoing Service Coordinator (OSC) for the Nassau and Suffolk County Early Intervention programs. This role is an excellent opportunity for both experienced professionals and new graduates looking to make a difference in the lives of families in the community. As an OSC, you will work in a hybrid environment with a flexible schedule, allowing you to balance your professional and personal commitments effectively. You will receive support from our knowledgeable Early Intervention team and ongoing training to enhance your skills and knowledge in the field. In this position, you will be responsible for guiding families through the Early Intervention process, ensuring they understand the services available to them. You will coordinate evaluations and services for children based on the availability of families and providers, ensuring that each child receives the necessary support. Additionally, you will participate in the development, implementation, and monitoring of the Individualized Family Service Plan (IFSP), maintaining regular contact with families, service providers, and NYTPS staff to ensure effective communication and service delivery. Your role will also involve monitoring services and maintaining quarterly progress reports, as well as conducting semi-annual IFSP reviews or amendments. Proper documentation of all contacts made with families and Early Intervention providers is crucial, as is any information pertinent to a child's case. This position requires a commitment to helping families navigate the complexities of Early Intervention services, making it essential for candidates to be energetic, motivated, and organized.
