About The Position

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Dundee Township Community Case Manager Mental Health Professional MHP/QMHP who demonstrates this mission and wants to work for an organization that makes a difference in the community. Schedule: Mon-Fri 8am-4:30pm • This is a grant funded position for the Dundee Community that incorporates supportive Case Management services for any Dundee township resident for temporary support with case management or therapeutic intervention services. • The MHP is responsible for the implementation of support service through referrals for continuation of services or assisting with allocating services to other providers to meet the needs for all residents in Dundee Township. • The MHP is responsible for the implementation of client’s IM+CAN treatment plan goals and objectives. The MHP demonstrates this through daily documentation of service provided to each client to whom they are responsible. The MHP will assure client rights and dignity while maintaining a clean and safe environment. Be familiar with and maintain all applicable program and agency regulations.

Requirements

  • Education: Bachelor’s degree in Human Services or related field required for MHP.
  • Experience: • Experience working with mentally ill adults or in case management preferred.
  • The use of personal automobile, a valid driver’s license, and the minimum amount of liability insurance as defined by AID’s Personnel Policy.
  • Must drive self and clients in personal or agency vehicles.
  • Must acquire and maintain certifications in First Aid, CPR, Non-violent crisis intervention training, CEU’s and other relevant trainings
  • Must be able to drive a passenger vehicle

Nice To Haves

  • Bilingual in Spanish preferred.

Responsibilities

  • Work scheduled hours and be flexible to meet client and program needs. Includes cross training and coverage in other programs as assigned.
  • Responsible to provide independent program coverage when scheduled.
  • Responsible for reviewing program referrals for Community Case Manager in Dundee Township, this includes verifying township residency, tracking contacts, data and outcomes of temporary services provided to area residents and program participants.
  • Update service tracker with referrals, time allocation for Dundee Township services and document collaborative outreach for local service providers.
  • Host Open office hours for walk in services at St. James Episcopal church in Dundee for those in need of drop in resources, referrals or support services.
  • Must meet minimum service hour provision productivity standards (MRO) monthly.
  • Complete and sign all MRO documentation (encounters or notes) every 48 hours in Cx360 for clients in the Supported Living Program.
  • Meet with all assigned clients on a regular basis per program and client needs; turn in activity logs on a daily basis.
  • Acquire and maintain required trainings and certifications as well as any other trainings assigned by Manager.
  • Provide effective services to meet clients in Supported Living Program individual needs in line with each IM+CANS and the Mental Health Recovery Model.
  • Maintain professionalism and good boundaries when working with clients, coworkers and outside agencies.
  • Attend monthly clinical supervisions per DHS requirement.
  • Attend monthly staff meetings and be a positive contributor.
  • Follow the direction of managers and case managers regarding appropriate client interactions and interventions per clients’ individual plans.
  • Assure compliance with all rules, regulations and recommendations per agency, state and federal guidelines. Review and follow updated policies and procedures.
  • Recognize emergency situations and take appropriate action. Contact manager and director per procedure. Complete necessary paperwork correctly (incident reports, petitions, encounters)
  • Promotes client wellness through coaching, counseling, consultation, motivational enhancement, and information and referral services to clients facing various health challenges, as needed.
  • Communicates with service delivery partners, which may include Physicians, Nurse Practitioners, Counselors, Pharmacists, Employers, Community Supports, and other Health Professionals, as needed, to facilitate the achievement of client’s wellness objectives.
  • Utilizes a variety of appropriate resources and educational materials for wellness coaching. (i.e., Health Matters Curriculum, Smoking cessation materials, Weight Management information, etc.)
  • Assure client records are properly maintained for clients in Supported Living Program per agency procedures including scanning necessary paperwork into Cx360 per procedure.
